Please read these Terms and Conditions carefully before accessing or using any services provided by Seraphin Medical and Wellness (“Seraphin,” “we,” “our,” or “us”). By scheduling an appointment, engaging our services, or accessing our website, you agree to be bound by these Terms. If you do not agree, please do not use our services.

1. About Seraphin Medical and Wellness

Seraphin Medical and Wellness is a cash-pay, concierge medical practice located in Nashville, Tennessee, offering aesthetic, wellness, and functional medicine services. Our services include, but are not limited to, medical aesthetics (neurotoxin and filler treatments), hormone therapy, weight loss programs (including GLP-1 and tirzepatide), gut health consultations, and functional medicine evaluations.

Seraphin is operated by a licensed nurse practitioner and complies with all applicable Tennessee state law governing advanced practice nursing and medical aesthetics.

2. Medical Disclaimer

The services provided by Seraphin are medical in nature and are performed under the oversight of a licensed healthcare provider. However, the following important limitations apply:

  • Services are not a substitute for emergency medical care. In a medical emergency, call 911 immediately.
  • Results from aesthetic, weight loss, hormone therapy, or other wellness services are not guaranteed and vary by individual.
  • Information provided during consultations or via our website does not constitute a physician-patient relationship unless a formal treatment agreement has been established.
  • All treatments will be preceded by an intake evaluation and, where clinically indicated, review of relevant laboratory results.

3. Cash-Pay Policy

Seraphin operates exclusively on a cash-pay basis and does not accept health insurance for any services. By using our services, you acknowledge and agree that:

  • Payment is required at the time of service unless otherwise arranged under a membership agreement.
  • We do not submit claims to insurance carriers. You are solely responsible for all fees.
  • Membership fees, treatment deposits, and package payments are subject to the cancellation and refund terms outlined below.
  • Pricing is subject to change with reasonable advance notice.

4. Appointments and Cancellations

Scheduling

Appointments may be scheduled online, by phone, or through our patient portal. New patient appointments require completion of intake forms prior to your visit.

Cancellation Policy

We require a minimum of 48 hours’ notice for cancellations or rescheduling. Cancellations made with less than 48 hours’ notice, or failure to appear for a scheduled appointment, may result in a cancellation fee. Repeated no-shows may result in termination of the provider-patient relationship at Seraphin’s discretion.

Late Arrivals

Arriving more than 15 minutes late to an appointment may result in rescheduling to protect the quality of care provided to all patients.

5. Membership Terms

Seraphin offers optional membership programs that provide access to discounted services, included treatments, and priority scheduling. Memberships are subject to the following terms:

  • Memberships are personal and non-transferable.
  • Monthly membership fees are charged automatically on your billing cycle date.
  • Included services within a membership period do not carry over to subsequent months unless otherwise specified in writing.
  • Memberships may be paused once per calendar year for up to 60 days with written notice. Pausing does not extend the membership term.
  • Memberships may be cancelled with 30 days’ written notice. Cancellation takes effect at the end of the current billing period.
  • Founding member rates and promotional pricing are locked for the duration of active membership and are forfeited upon cancellation.

6. Refund Policy

All sales of treatments, packages, and memberships are generally non-refundable. Exceptions may be made at Seraphin’s sole discretion in the following circumstances:

  • A treatment cannot be performed due to a clinical contraindication identified at the time of service.
  • A service error attributable to Seraphin is documented and verified.

Refund requests must be submitted in writing within 30 days of the service date or billing date. Approved refunds will be processed within 10 business days.

7. Informed Consent

Before any treatment, you will be asked to review and sign an informed consent form specific to the procedure being performed. Informed consent includes discussion of potential risks, benefits, alternatives, and your right to decline or withdraw consent at any time prior to treatment.

By proceeding with any service, you confirm that you have disclosed all relevant medical history, medications, supplements, allergies, and conditions to your provider and that the information you have provided is accurate and complete.

8. Intellectual Property

All content on the Seraphin website and in patient communications, including text, graphics, logos, treatment protocols, and pricing materials, is the proprietary property of Seraphin Medical and Wellness. You may not reproduce, distribute, or create derivative works from our content without prior written consent.

9. Limitation of Liability

To the fullest extent permitted by applicable law, Seraphin Medical and Wellness, its owners, providers, and staff shall not be liable for any indirect, incidental, special, or consequential damages arising out of or related to your use of our services. Our total liability for any claim shall not exceed the amount you paid for the specific service giving rise to the claim.

Nothing in these Terms limits liability for gross negligence, intentional misconduct, or where limitation is prohibited by law.

10. Governing Law

These Terms are governed by the laws of the State of Tennessee without regard to conflict of law principles. Any disputes arising under these Terms shall be resolved in the courts of Davidson County, Tennessee.

11. Changes to These Terms

Seraphin reserves the right to update these Terms at any time. We will notify active patients of material changes via email or patient portal notification. Continued use of our services following notice of changes constitutes acceptance of the updated Terms.
